Step 1:
Complete Your
Secure Intake
Getting started begins with a secure online intake. You’ll provide your medical history, current medications, prior weight-loss experience, and goals so your Nurse Practitioner can evaluate whether treatment may be appropriate.
Step 2:
Telehealth Visit With
Your Nurse Practitioner
After completing intake, you’ll schedule a telehealth visit with your Nurse Practitioner. During this visit, your history is reviewed in detail, medication options are discussed, and eligibility is determined based on safety and clinical guidelines.
This visit focuses on education, expectations, and shared decision-making — not pressure or one-size-fits-all recommendations.
If clinically appropriate, a personalized treatment plan is created based on your medical history, goals, and preferences. This may include GLP-1 medications, oral weight-loss medications, or non-medication recommendations when indicated.
Step 3:
Personalized
Treatment Plan
DISCLAIMER: Medication is prescribed only when appropriate and is billed separately by a licensed pharmacy.
Step 4:
Pharmacy Coordination
& Medication Delivery
Once a prescription is issued, it is sent to a licensed partner pharmacy. The pharmacy will contact you directly to confirm payment and coordinate shipping. ClearPath Health & Wellness provides medical care and prescribing services only. Medication costs are handled separately by the pharmacy..
Care does not stop after your first prescription. Patients are followed regularly with telehealth visits to review progress, adjust dosing when needed, and address side effects or concerns.
Step 5:
Ongoing Follow-Ups & Monitoring
What Makes the Process Different
ClearPath is intentionally structured as a Nurse Practitioner-led practice. Care is managed directly from start to finish, without hand-offs or fragmented communication. This allows for thoughtful decision-making, continuity, and long-term support.
Important Safety
& Expectations
Weight-loss medications are powerful tools and are not appropriate for everyone. Results vary, and treatment plans may evolve over time. Ongoing communication, follow-up, and education are essential parts of the process.
