ClearPath Health & Wellness provides telehealth-based clinical care focused on weight-loss and metabolic health. Services may include medical evaluations, follow-up visits, messaging support, and treatment planning by licensed healthcare providers.

ClearPath does not dispense or sell medications directly. Prescriptions, when appropriate, are sent to licensed third-party pharmacies, which handle medication billing, fulfillment, and shipping independently.

You must:

Be at least 18 years of age

Be physically located in Florida at the time of care

Provide accurate, complete medical and personal information

ClearPath reserves the right to decline or discontinue care if clinical criteria are not met or if required information is incomplete.

All medical decisions are made between you and a licensed provider following an individualized clinical evaluation. Results vary, and no specific outcomes are guaranteed.

4. Telehealth Consent

By using ClearPath’s Services, you consent to receiving healthcare via telehealth technologies, including video, phone, and secure messaging. Telehealth has inherent limitations, and you acknowledge that in-person care may be required in certain situations.

5. Payments & Billing

ClearPath charges fees for clinical services only.
Medication costs are billed separately by the pharmacy and are not included unless explicitly stated.

Fees, billing terms, and cancellation policies will be disclosed prior to enrollment or during intake.

Follow prescribed treatment plans

Failure to comply may result in discontinuation of Services.
